At least annually, drivers must prepare a list of all violations of motor vehicle traffic laws and ordinances (other than parking violations) of which the driver has been convicted or has forfeited bond or collateral during the preceding 12 months. Additionally, carriers must annually review each driver’s motor vehicle record and Certificate of Violations, as well as other information described in 49 C.F.R. 391.25, and complete an Annual Review form to place in the driver’s file.


This document serves as both the annual Certificate of Violations and Annual Review of Driving Record.
